This has The Island of Dr. Moreau written all over it. Maybe do a next generation story, where the doctor is the daughter of the original. Give it a harem spin where transformed are the doctors lovers.

Got another idea:

World where recreational transformation is starting to happen but it frowned upon.

MC is a guy who got into it as a supplier as his degree was going to waste. He mostly gets curious people just wanting things like “a bit more height” “bigger dick” etc. it pays the bills but it’s not satisfying.

Then brother’s girlfriend finds out about what he does and asks if she can come by. She admits brother was skeptical until she said she could get bigger natural tits…but she also wants to try something a bit more unique but simple: a tail that has an eye on the end.
