If it ever hurts, it hurts so good.

I've only had one 'not good' experience with a steel cock ring and that was due to an extended wearing period. The ring was a little too small for being around my shaft and it left a dent that was there for a couple days (yes, there was a dent around the entire cock).

The leather only rings don't ever really hurt and if they did, they could easily be unsnapped or unbuckled.

The ring in the picture you saw at the beginning of this thread is one that can be locked on, the lock is usually on the other side of the cock though - so you don't see it in this picture

A lot of people have suggested that you shouldn't ever use the steel rings, but if you've sized them correctly (like the ones I use are) then there isn't much risk of pain or damage. There is, however, a risk of the cock ring coming off, rolling down your pantlegs and onto the floor in very cold environments. Shrinkage is bad.
