She did not sign the OF-109...big problem for State Dept.

Big problem for Department of State, but not for Clinton it seems.

note also that OF-109s from Colin Powell or Condi Rice are not on file at the State Department either.

It can't be her responsibility to make sure that she fills out the proper separation paperwork. Although, that would explain why there are no forms on file for Colin Powell or Condi Rice either.

Maybe it should be someone's job to process outgoing official's paperwork to make sure everything is done.

Why isn't it her responsibility to see that her department obeys it's own regulations?

How asinine would it be to expect someone leaving a position (any position) to ensure that they are compliant with separation paperwork? Companies have Human Resources to ensure that all of the necessary paperwork is completed. Is it your assertion that the State Department has no such entity?

The fact is, the person responsible for collecting the records, whoever that may be (probably someone from the National Archives), is the one who is responsible for getting the form signed at the time of collection.
