Hacked Account?

Is there any way to find out if my account has been hacked?
I've been suspicious for a while. Over the last few months, I've noticed when I get to the end of stories I'm absolutely sure I've never read, they are already marked with a score.
This morning, I was looking back at one of my old stories. As a matter of ethics, I never score my own work, but this one suddenly had a score.
Scoring is the only symptom I've noticed. Can we find out if someone is logged into my account from different addresses? I connect from only one device at a time, until I replace it.
Could this be a weird bug in the system, automatically or randomly assigning scores?

This is largely a community supported forum. Whether or not your account has been hacked is virtually impossible for us to say.

I changed my password last week. I don't know when my story got scored using my account. I will presume from your response that no one else has reported a similar issue, so I'm hesitant to push anybody to spend time debugging something that is probably not there. I'm surprised the database engine doesn't have a tool to trace user connections by login.
