Since this is fiction, I would hesitate to pin down the drug to anything specific in the real world, but certainly it could and would act something like Viagra -in that, because an erection is caused by increased blood flow, you can create this effect chemically. This is part of the reason I mention the main character's racing heart, because I think such a drug would have that kind of effect in addition to his induced hard on. I would also say, with all this being the case, it is entirely realistic for the main character to remain erect and aroused despite his mental state of mind -indeed, that is where part of the tension of the story lies, in the disconnect between his circumstance and his erotic state of need.
