I need a comprehensive history ... so ... when the fetish is known to have developed, the general psychology surrounding the fetish, anything regarding the sociological implications, especially when an obsession for a professional occurs, or when a professional actually crosses the line (anecdotal). Lifestyle aspects are easier to find, but all relate to only one aspect at a time ie: role play, spectulum, catheterization or enema. I need to define medical fetish in the broader context.

Perhaps answering this question:

What is "particular" about medical fetishism and how does it differentiate from role play? Would be greatly useful to me.

You're welcome. The only other thing that comes to mind is possibly contacting authors who have written stories that seem to have a good grasp of the topic (I've read several like that; maybe try a story search with "doctor"). Someone has to know, or at least point you in the right direction.

Would you mind sharing your purpose/what you're going to do with the info? Your questions have piqued my curiosity.
