I've designed a couple of dozen different ones. Don't worry - the only people who dread them more than staff employees are the managers responsible for administering them.

Amen! I hate having to do these things. Especially on forms that have nothing to do with the jobs I'm evaluating. Our Human Resources people aren't always helpful enough to have different forms for people in operations vs. the admin side of the business. Evaluating office workers on their safety records, the neatness of their "tools", etc. just loses something in the translation. Especially when when you have a scale of 1 to 5 to mark a number, and that's it.
