Not a lady, but one of the theories I have heard is that it may has something to do with the distance between the clit and the vagina, and to an extent the shape of the labia. That essentially some women are getting clitoral stimulation from penetration through the combined movements of the vulva. The farther they are the more skin elasticity attenuates the 'signal'.

The vast majority of my experience has been with two ladies and they both orgasmed from PIV, so I'm lacking in data points in the other direction. (And yes I'm sure. The latter had a longer refractory period than I did, which... created problems)
