The short version:
About 7 years ago, my wife and I were staying with friends, and "indulged" in the middle of the night. As we were in a strange house, I didnt want to get up in the middle of the night and disturb anyone, so I didnt go to the bathroom (an absolute must if you have unprotected anal, is for the guy to pee afterwards to flush the urethra)....
A week later, I developed a hideous bladder infection, which hospitalised me for a week. Had I not been given IV antibiotics, the infection would probably have killed me....
It took a further three weeks to recover sufficiently to return to work....
However, shortly aftewards, I was diagnosed with an enlarged prostate, "prostatitis", which required some fairly invasive procedures to both diagnose and treat. Thankfully, once I received the right treatment, the problem was easily sorted, and after a course of drugs (I cant remember the name) I am back to normal.
Now, we use condoms, every time, no excuses, no messing about.

If you do forget to use a condom (and in a 1-1 monogomous long term relationship, it does happen) then make sure right away afterwards you go for a pee, and wash yourself really well..... Actually, it helps if you down a pint of water before making love..... so you have plenty to push out of your bladder to flush out your urethra....
Fairs fair, look at some of the preparation that women go through in their pursuit of anal pleasure... drinking a pint or so of water before hand pales in comparison !
