I suppose it really depends on what you started with. Plus there are those people who are born with ambiguous genitalia as well, or people with certain gland or chromosome issues where they may look fully male or female at birth but have real biological reasons to identify with the opposite sex. In general, our society has quite a narrow view of gender. Your male or female, gay or strait. I think its more of a continueum, and gay and strait are pretty much social constructs. Male and female is real, but its not all there is. Most people can't seem to deal with the fact that there are natural variations, or they just don't know. I think its all quite fascinating.....
