That skin, the frenum, was much like getting my ear pierced - not that big a deal. Now the jewelry I usually use, not the wand, rubs on that underside area and feels good.

Back to the original poster's general question about chastity:
The CBT-2000/-3000/The Curve line really work well if you fit them properly. The actual "cage" portion is probably the least important part of these devices. The ring that goes behind the scrotum and the length of the alignment and locking shafts is more more important, both for comfort in long-term wear and for security. The Curve, I know from first-hand experience (I'm wearing it now) comes with a set of variable-length spacers for this purpose. In addition, you can now buy accessories called "Points of Intrigue" which replace some of the spacers - they have very sharp little inward-pointing teeth that poke into the skin and help prevent intentional removal and (happy by-product) make arousal even more painful than just the cage alone.
Goddess has required me to wear mine for up to three weeks at a time. Currently I'm on my fifth day since getting the Points of Intrigue accessory and it makes a very big difference - I'm MUCH more aware of my enforced chaste state when the slightest arousal reminds me - painfully! - that I have a keyholder.
