Need Motivation

My one issue is that I tore my ACL about 10 years ago. And well, my knee hasn't been the same since even after rehab.

Ah.. that's a problem if ever there was one. Swimming might be your best option.

One important snippet - habit is stronger than willpower.
So rather than trying to force yourself through self-discipline, maybe just focus on adopting healthy habits, rephrase things mentally so you're thinking: This is what I do instead of This is what I want to avoid doing.

I keep myself focussed by using a spread sheet with my daily weight logged and graphed. Progress is slow (age-related slowed metabolism) but it's steady. Yesterday I even used statistical analysis to prove to myself that my weight is genuinely declining.
