My suspicion is that you need a bit of training to get there. You're used to a certain mental and physical stimuli to reach orgasm, and while nipple play is a wonderful prelude, you've not reached the mental state to push it over the edge. It's kind of like masturbation, you know what you like and can bring yourself to orgasm quite easily and quickly, but when you have a partner it's not so easy, is it? BUT, when you reach the comfort level, familiarity required, you will probably experience success!

You might want to try depriving yourself of an orgasm from anything OTHER than nipple play.
