The breasts are the most shocking part, and I know I've seen breast torture that extreme before. I certainly wouldn't want it done to me, but I could see it happening in real life.

Nope, everybody I know of has retained their mammaries! It would be pretty hard to castrate breasts in that way - it's hard enough to castrate balls using an elastrator band, you have to leave it there for a couple of hours at least, and that is MUCH tighter than this. (By the way, that link has some shocking pictures...not for the squeamish.
