A good friend of mine for example, had intensive counselling after the breakup of an abusive marriage. She is a devout Jehovah's Witness and made it clear that she wasn't interested in being told that atheism is the answer while she's vulnerable and uncertain of everything. Her therapist respected her personal beliefs at every turn because ripping the rug out from under people when they're mentally unwell, questioning the things that give them security and the fundamental basis for their daily lives, is a very bad idea.
If a person is strongly kink orientated and it forms the basis of their sex life, their norm as it were, then any sexual therapy would have to work within that context to be effective IMO.
