A guy with split personality disorder, like Tyler Durden from Fight Club, finds out that one of his alter egos has been fucking his wife behind his back. He finds out it and confronts the culprit only to be completely blocked by him whenever his wife takes off her clothes. First, he only blacks out and finds himself waking up in different places, places he's never been. Then, he sees the guy fucking his wife like a ghost in an out-of-body experience. You can also include some dramatic plots in which the villain cheats on his wife with other girls, who are equally made inaccessible for the MC. Eventually, the MC settles for the role of a cuck with the promise that the villain will give him some scraps after his loving-making sessions with the wife.

Honestly, I was expecting time-travel, but I really like this concept, too.

If he's not realizing it's his own body cucking him, maybe he decides to get back at his wife, the alter ego (or egos) goading him into having sex with someone else to prove he's a real man. Completely self-destructive, maybe dissociation triggered by something, like he found out his wife was actually cheating, but it was one time. It so broke him he dissociated and forgot, but now sees this alter fucking his wife, not realizing it's himself. He's pissed, but is also too scared to leave the marriage, so he's goaded into getting revenge by cheating.

When he tells her what he did to get back at her, she's shocked. He tells her he saw her having sex with the alters, and he knew that they were even filming it. When she shows him the tape, he realizes it's himself all this time.
