Giving head?

I've wondered about this for a while, but what is behind the phrase "Giving head"?

Does "head" refer to the head of the penis? If so, what does "giving" have to do with it?

Or does "head" refer to the head on the shoulders of the woman who is "giving" it in order to please her man?

Or is there some other explanation?

I suspect it's due to shortening the phrase "giving a head job" which does of course involve the head of the penis and so on.

I often wonder what a lot of things mean...in this case..who cares ..as long as it is good head.........
