If a person gets a kick out of hurting others, and is good at it, no reason you can't have empathy and be sadistic. If they're not resonating emotionally, then maybe there's little empathy, but they could be very perceptive - a clever asshole. Lots of different ways to pick up on what someone else is feeling without actually recreating some part of those feelings in yourself.
