Very vague concept, almost worthy of a Plot Bunny Pen, but, here we go...

whenever the old player piano is played, it plays the song that grandpa used to seduce grandma. The effect is that the family member older than but closest to 18 instantly has the ability to seduce the person they are with...

Make it something smaller and more intimate. Something that can be given as a romantic gesture. A music box. Older ones had removable cylinders to play multiple tunes, like a player piano. Plus the music is more haunting and surreal from a music box than a piano in my opinion.

reminds me of that Twilight Zone episode where a guy gets a player piano that, when depending on the roll, reveals secret sides, inner selfs or changes a person’s temperament.

Maybe someone finds out about the effect but someone decides to switch the roll to teach them a lesson. So instead of being able to seduce anyone, they can be seduced by anyone.

Maybe as a bit more heartwarming ending once the punishment ends. person who switched the roll then reveals while there is a roll that has that effect, grandpa actually played a different roll: one that made it so he couldn’t hide his feelings from grandma.
