Generally it's simply electricity that heats up rocks that retain and give off heat. Sometimes you have steam rooms that you scoop up water from a bucket and pour it over hot rocks and steam is produced.
Normally what I have seen guys will go in there wearing their gym clothes. I think they would prefer you take a pre sauna shower and go in there with a towel wrapped around your waist and keep yourself covered. I suspect at larger gyms with larger saunas you would have lots of guys in there exposing themselves, but it's not like they are having sex. I have heard about, but never been to one, very large spas where guys head to their guys only pool and ladies go to their ladies only pool and they are fully naked pools, no clothing what so ever allowed.
