I'd say she knows exactly what she's doing. To me it looks very controlled and the effect is to make it look intense an painful when in reality its not. If you watch carefully the top of her foot connects with the back of his sack. His balls are flipped forward and upward colliding with only his penis before swinging back down between his legs. That's much different than a knee or foot to the groin which crushes the balls against the pelvic bone. I'm not saying she's not making him suck in his breath and see stars, but I don't think he's being paralyzed with pain.

The highlighted part is the reason I think it belongs only to the web. Real life participant would not care about how it looks -- he can't see it from the side. For him it is only about how it feels. I agree that it is very unlikely they got an actor paralized with pain, but it is more than likely that he did not feel much pain at all. Which kind of defeats the purpose of the whole thing.
If you are going for the foot fetish, then why the kicking?
If you are going for cbt, then do someting with much better controlled pain than that.
If you are going for the fear factor, then there needs to be clear anticipation of the action and there is none here.

True. It's safer to kick, slap, or punch unbound testicles in such a manner that they're not crushed up against the pelvic bone. Of course, it's always possible to sustain an injury with a hard-enough impact, but the likelihood would be lower. (I do have a bit of experience on the receiving end ;) )
