A friend of mine just got back from Japan, he spent a couple of months there and said the cities he visited were the cleanest he'd ever seen. He said that the Japanese people were friendly and helpful but he felt they were a very parochial people and only tolerated Westerners.
Major cities are completely westernized to accommodate tourists. It's said that if you get off the tourist routes and go into the hills you'll find that the people are still polite but are much more reserved. Which translates to seeming to be less friendly.
