Married for 30 years. Haven't had sex with penetration since August 2012. Oral sex once since them (with me receiving as well as giving); twice more with me only giving.

A couple of your points caught my eye:
1. Her jaw gets too tired to give blowjobs. Poor thing is doing it wrong. Most women get their techniques from cheesy porn, but don't seem to notice that stock footage is looping to make the scene last longer. Next time, guide and complement her on using more licking and hand technique. Also, have her French kissing the head while using a moist hand to stroke the shaft. By using her hand to extend her mouth, it saves wear on the jaw.
2. Penetration is painful. There are 3 likely causes. A) you don't have sex so the stretching is painful, so she avoids it, thus continuing the problem. Solution is to pleasure her more by using fingers inside her. B) she suffers vaginal dryness. This is almost always a hormone problem. She may be old enough for menopause symptoms, and uncomfortable dryness is one. The synthetic hormones in birth control are known to cause all kinds of problems, including dryness. Try a gentle lubricant. Also, side effects from hormonal birth control can linger for years. Just because she's off the stuff doesn't mean no more problems. C) uterine fibroids. These are non cancerous growths of non useful tissue around the vagina and uterus. They can tighten a vagina by basically pressing on it and not allowing it to expand to accommodate a penis. Since they are non cancerous, doctors are damn lazy about treating them. The can cause everything from anemia (due to extended and heavy menstrual cycles) to migraines to fertility issues. If it interferes with fertility, a doctor will consider removing the fibroids, otherwise all he will offer is a hysterectomy (uterus removal which causes instant menopause). Removal of the fibroids is considered unnecessary. A woman can get a boob job or nose job any day of the week, but she's told to live with fibroids that make her life hell

Not enough. We were averaging about twice a month. He has stepped it up a bit and I get it maybe once a week now. Hey, anything is an improvement, right? Only problem is I don't even really want it anymore. Irony.

I am not yet married but have been in a relationship for a while and I have to be honest, this thread makes me a little leery about getting married or the quality of sex life I can expect to have down the road...

You're very wise to be leery. There are a LOT of fucked up marriages out there, believe me. Don't fall into the trap of getting married because it's what you're "supposed" to do. Think long and hard about it. Just my $0.02.

Oh, don't get me wrong. I want to fuck all.the.time. I just have spent nearly 20 years having him turn me down or push me away so something finally clicked and I don't feel particularly amorous towards him. You're right...there is definitely someone I'd enjoy having sex with. And when hubby and I have sex I enjoy it, I'm just not really interested in having sex with him. It's almost like soda. I like soda well enough but I don't buy it or crave it or want it but if it's all there is to drink I like it well enough.
