No, I haven't. I have seen it though, and it hurts!

I had a friend one day (I was right beside him) who was running on the treadmill, pretty full tilt. Suddenly, and completely without warning, the tradmill simply stopped. The only reason he didn't go completely right over the front was that it basically up against a wall. He face planted the wall, and bounced back down. He was in really good shape, and way more coordinated than I, which probably saved him from serious injury.
