I don't think he's dumb enough to not know about circumcision, but that's not what he's asking anyway...he's asking how he can make the head larger and more mushroom-shaped. Circumcision is a whole different ballgame, and had he said, "I'm uncircumcised and I'd like to change how my cock looks." someone probably would have suggested consulting a physician regarding the possibility of circumcision.
The difference between penis augmentation and other cosmetic or reconstructive surgery is vast...you can't compare them. For one thing, the procedures you've mentioned have been performed over and over for a very long time. They have been refined, there is a lot of knowledge about them, and they are relatively safe as far as surgeries go. Furthermore, they are operations on parts that are not packed with nerve-endings, can handle scar tissue relatively well, and whose functions are not so essential to quality of life. None of that is true for surgeries on the penis or clitoris (excluding circumcisions and other procedures that are relatively safe and commonly practiced). Penis enlargement surgery isn't done by many doctors, and is borderline safe...there's a huge risk of complications, and they occur frequently. Overall, it's far better to live with the genitalia you have than risk not having anything at all.
