I can't speak to the proportions of the various slants to the fetish, but I was always the one doing the peeing. Son I suggest it definitely can go both ways, and no doubt some like it both ways as well.

This starts a summertime story with POV switching between the Girl, the nephew, and his uncle. I figure uncle is initially introduced as a dirty old man, and he is but he also is looking out for the girl. Once she found her fetish, she got a bit too enthusiastic so he’s taken a pseudo dom role, making sure she’s exploring it in a safe way. Meanwhile nephew is both shocked but also intrigued: uncle recognizes he could stand to figure himself out so he lets nephew also try out the fetish. Switching POv is tricky to write (and some readers don't care for it because it can be confusing). I suggest considering a third party POVas a simple means to reflect the various characters and their thoughts .

My first thought was the jellyfish thing, too. But in my mind, I can see a guy who is into doing the peeing, so he starts hanging out at beaches with high jellyfish populations, and always volunteering to provide the treatment. He starts to notice one girl (or maybe a guy) seems to get stung a lot and therefore needs lots of treatments. Until he figures out that she is really just into getting peed on.

My other idea was a bunch of frat guys having a kegger. Of course they all have to pee as they guzzle their brew. They are outside in the frat house yard and it's a long walk to the bathroom (or maybe they are on a camping trip), so they mostly just pee in the trees near the fence line.
