I was just wondering if it is possible to catch an STD from a toy that has been washed with just water before use?

Hmm. Someone will come along with a better answer, but I would say, put it in bleach to be sure, hehe.

What if the toy has already been used and was only washed with water?
Do you think its possible that she has contracted something? Not so much HIV but Chlamydia, Gonorrhea, or Syphilis.
The toy has already been used is she at risk?

Yes, it's certainly possible, and sharing anal toys especially is never adviseable. The woman you're speaking of should probably see a doctor or go to a clinic, explain the situation, and be tested for everything. If nothing else, it'll give her peace of mind. Then she should read up and talk to the doctor about the risks, cleaning, and general safety.

Edit: A couple of results from googling 'sex toys std'
