One is about to die. Medical science cannot save them.

But it has an alternative. Doctors have engineered a way to implant the consciousness of the soon to be departed into a living host where both souls will occupy the body equally.

Author gets to determine who the souls are and what activities they get each other into.

This is not SciFi in the sense of far future or another world. This is modern day, normal life like Bewitched, Jeannie, Uncle Martin, etc.
 
that’s effectively the plot of “Get Out”

But let’s make it slightly less horrifying.

So rich guy is about to bite it and he wants to hold on for a bit longer. So after checking with his lawyer to make sure there won’t be any legal issues like his will paying out due to his medical death, he finds a poor but healthy guy who seems like a doormat as that will let him have less argument in the body. Less fighting for control.

Guy agrees and it goes great! The guy’s there but he doesn’t impede him so the rich guy is free to live it up, run his company, etc. he’s also in a legal situation where he counts as two people.

Then one day, while he’s preparing to board a plane, he suddenly finds himself locked in a small room made of cinder blocks.

See, the host wasn’t a doormat. He let the old guy take the wheel so he could learn all his passwords, get the biometrics traded to his fingerprints and face, and learn The Guy’s behaviors. Now he’s locked the rich guy in a mental prison he’ll never escape from while he gets to enjoy the rich guy’s current wife and ex, who paid him to arrange it all. And if the rich guy makes too much noise…well, one trip to the surgeon and out he goes
 
But let’s do something less horrifying:

prisoners get combined during the testing phase. In exchange for commuted sentences one of them dies and the other takes them.

It’s an old guy (they didn’t take anyone off death row) and a younger guy. Of course the scientists aren’t interested in their well being aside from how it effects their physical health but the two form an odd friendship. As they spend more time together, they soon find themselves merging into a new consciousness
 
Problem is I don’t know how to make it erotic aside from pseudo cucking by the host fucking the symbiote’s partner.

I think it works best for dramatic stories:

Woman takes a younger woman into herself. She volunteered but the truth is she fudged some records: younger woman killed her daughter (not in a way that got prosecuted). She intends to make life hell for her…but now they’re linked and she gets to see this woman’s POV about what happened and the young woman get’s to see her POV) the two of them have to figure things out.

How about this...

One person (leaving the gender open for whoever writes this) is in medical jeopardy. Of course, they have money...some level of power...yada yada.

Suddenly, it's become an emergency and they go into a coma before the symbiote was chosen. Doctors have to make a choice. The only available, medically compatible symbiote is a member of the other sex. Said symbiote was in some sort of medical storage, their body long gone to whatever wasting disease they were dying of.

The transfer is complete and the first person is healed and wakes up from their coma with the second person inside. They have to learn how to accommodate each other and how to mesh their sexual desires and preferences.

Nothing wrong with a bit of Erotic Horror on Literotica. it doesn't all have to be cutesy vampires and werewolves. I'm confident that there would be a niche audience for this.
 
I think the tricky part for me is the “equal control” bit. Like imagine two people trying to drive and all the ways they could trip over themselves.

I think maybe the way to do it would be for the host to allow the symbiote to take control or for them to sort of merge into a new entity that is more the host but has all the memories of the symbiote so they are somewhat them to (like the Trill in Star Trek)

The other way would be for it to be a temporary thing. Like imagine if someone in a coma could be transferred. They could check if the person is still there and then perform more radical treatment while allowing that person to continue their life by having agreed upon time where they would be fully present

Idea: Nun volunteers to play host to a parishioner who was in a terrible accident. The nun’s motivation wasn’t entirely selfless. She was secretly in love with the parishioner but never knew how to express it. Maybe they take turns masturbating as an indirect form of love.

For extra complications whether parishioner accepts her love or not, she was married and had a family. Of course husband doesn’t expect nun to have sex with him…but when he hears his wife’s words through the beautiful nun’s full lips and is being led upstairs…he isn’t exactly going to say no even when his “wife” starts acting really shy or has a completely different technique than before

Two examples of symbiotes come to mind that could fit this description without stretching too much, both from settings that are technically modern Earth but are still a lot more out there than your examples: Stargate: SG1 and the Marvel Universe.
  • Stargate: SG1: there are these aliens called goa'uld. They are the main villains of the series, but there are also some non-evil goa'uld called tok'ra. They can enslave people and leave the host no control, but they can also make it a mutual thing. Their life cycle is complicated, but they get their hosts' memories and vice versa, so a writer could make an alien like them have a very humanlike perspective if they wanted. They can also slightly enhance a human's health and physical abilities, so you could enjoy body modification fantasies there but overall it would be pretty grounded besides the basic two-minds-in-one-body premise.
  • Marvel Universe: Spider-Man's frenemy Venom is the main example. The species is called klyntar. Memory-sharing is rare if not nonexistent, but they can talk to their host mentally. The symbiotes can't generally control people or change their host's body, just add on to it, but can make them a lot stronger and also give them tentacle-like growths. That's a completely different fetish.
IMO the most interesting part of the idea is genderblending. Maybe a tok'ra starts in a woman's body and jumps to a man's or vice versa, and brings some memories with them? Maybe a woman gets a klyntar and enjoys being the partner doing the penetrating, even if the tentacles aren't actually erogenous zones?

Jordan Peele is as good at directing horror as he is at sketch comedy. It’s a good “wtf wtf wtf” horror. There’s a little gore but it’s mostly psychological.

Unless you avoid the genre entirely, it’s worth your time. I watch maybe 1,2 scary movies a year and I watched Nope because of Get Out.
 
What if there’s a retrieval mechanism, where your host is temporary while they grow you a new body and the body takes a long time, so if you don’t know you’re sick soon enough you might die in the interim. Or maybe theres a 1% failure rate for the clones, so you have to wait for a second.

Instead of an intruder or usurper, you’re a tourist in this person’s life. You are literally a fly on the wall as they go about their experiences.

So maybe you’re there to watch them make out with women, men. Maybe you want physical adventures (eg, you’ve been in a wheelchair for years and you want to figure out what you’ll do with your legs once they work). Maybe you even kicked in extra money for the ‘rental’ so they could engage in some wish fulfillment.

Or maybe you see all the little kindnesses they don’t ask anyone to acknowledge and you slowly fall in love with them over the course of your stay.

If you know you’re being watched you’ll try to curate what the other person sees, but things sneak through. Especially when the other person has absolutely nothing else to do.
