The notion that people in the 1500's had even a clue about the source of diseases is ahistorical in the extreme. They typically blamed foul smelling air and evil spirits for their diseases.
Lepers were made to leave populated areas because leprosy was seen as a curse from God in punishment for sin. Again, this had fuck all to do with any understanding of disease prevention and everything to do with superstition and ignorance.
