horizontal - needs very little hood skin to be done successfully.
vertical - needs about 50% more than the minimum for a horizontal (if it ain't there, it won't stay there)
regarding triangles - can be done to almost everyone, but needs a VERY experienced piercer, as the risks to clitoral nerves are far greater with this piercing than with the previous 2.

I haven't had it get caught in clothing though I am told by friends that have the ring that sometimes that gets caught. Since I have the barbell, the same one you would get if you had a navel piercing, I have no problems.
As for orgasm problems and re-learning your body, I'm told that sometimes, but in rare cases, having the actual clit pierced can be damaging. My piercer was one of the folks that mentioned this to me which is why when he said "Maybe you should get the hood done instead..." I said "Yup...sure will, thank you."

Interestingly, my cousin was in an accident years ago and had several operations on his ankle and promptly contracted Golden Staph in the hospital.
