I think 99% of the time it's not accidental but when there's a lot of lubrication involved, either natural or otherwise, and activity is sufficiently vigorous, it can definitely happen. I know that firsthand. A long time ago exactly that happened to me and I realized it after about ten seconds. Of course she knew immediately but didn't say anything. I found out why a few seconds later. So I said "Oh my God, I'm sorry!" She just looked over her shoulder, smiled, and said "It's okay. Stay inside. Go harder." Not sure if most people fall into the category or not, but I've never been a huge fan of food play. I like the way a woman tastes naturally and whip cream, chocolate, etc. have never been anything that gets me excited seeing it coated on a woman.
